Responsible for coordinating and performing intravenous medication administration to patients in the home setting, providing patient education on home infusion therapy administration prior to discharge, and supporting pharmacy operations to service home infusion patients.

  • Successfully instruct patients in pre-discharge education on home infusion therapy self- administration.
  • Provides direct nursing care in patients home for the administration of home infusion therapies and troubleshoot patient infusion concerns.
  • Provides, coordinates, and directs the provision of home health nursing services and case management processes according to the assigned patient's individualized plan of care based on agency policies and procedures, through the competent application of nursing process.
  • Take on call responsibility as assigned to provide 24-hour availability and provide these services in accordance with established protocols.
  • Provide clinical episode management according to Amedisys' processes including oversight of patient's individualized plan of care and care plan changes as deemed necessary by patient's status or physician's ordered care
  • Attend scheduled patient care conferences, in-services, departmental meetings, etc.
  • Communicate to supervisor, physicians, and other health care providers, as appropriate, any changes in the patient's condition.
  • Complete assigned work on schedule, including all required patient care documentation and reports in a timely manner.
  • Maintain confidentiality of patient information.
  • Maintain professional licensure, certification and health requirements as well as demonstrating clinical competence of orientation programs, etc.
  • Utilizes a combination of agency resources and nationally recognized standards of practice to deliver high quality care and achieve excellent patient specific outcomes.
  • Promotes and maintains patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques. Advocates and ensures patient participation in care planning and goals of care. Utilizes accepted Amedisys teaching materials for clinician and patient/family education.
  • Completes documentation timely, accurately, and at the point of care, according to industry standards, conventions, and guidelines, including OASIS assessments, SOC/Admissions, Recertification's, ABNs, subsequent visits, physician orders, care coordination etc.
  • Helps to achieve and maintain continuity of patient care by communicating information (for example, changes in plan of care, new orders, lab results, etc.) timely, effectively, and systematically with all team members providing care for the patient including PT, OT, ST, MSW, paraprofessionals, physicians, family members and community resources. Facilitates interdisciplinary communication and follow up through care coordination on assigned caseload for duration that patients are on service.
  • Remains knowledgeable of organization's quality improvement programs, participates in agency performance improvement initiatives and incorporates into patient care planning as appropriate.
  • Initiates, plans, and provides staff education, and performs competency and skills validation for care team staff, such as LPN's, HHA's and RN visit staff. Participate in the orientation of other nurses or center personnel, as requested.
  • Take professional responsibility for self-protection during required work in potentially hazardous situations (i.e., handling of cytotoxic agents, handling of blood and body fluids).
  • Support the marketing efforts of the center by assisting the liaison team and providing educational programs to referral sources, as requested.
  • Comply with all applicable company policies and procedures.
  • Participates in clinical development and continuing education programs.
  • Performs other duties, as assigned.

  • Diploma from an accredited school of nursing.
  • Current, unencumbered license to practice as a Registered Nurse specific to the state(s) the employee is assigned to work/cover for the company.
  • One (1) year clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical. One (1+) year clinical experience involving infusion therapy, including patient assessment skills and venous access skills
  • Excellent peripheral IV insertion skillset.
  • Current CPR certification.
  • Demonstrated knowledge of the appropriate skills for communicating with individuals of all ages.
  • Excellent interpersonal skills including verbal and written communication skills.
  • Demonstrates knowledge of Federal/State rules and regulations, ACHC standards, and other regulatory requirements.
  • Valid Driver's License.
  • Reliable transportation and agency required liability insurance.
  • Must be familiar with general use and functions of the computer, such as, usernames, password concepts, internet, email, navigation of computer desktop, including starting programs, using files, and windows, effective use of navigation buttons and tool bars, ability to self-manage online HR services and online training programs

  • Bachelor's degree in skilled nursing.
  • One (1) year previous specialized home health experience.
  • PICC training
